Yamashita等[51]2021年在side overlap吻合的基础上进行改良,并命名为 modified side overlap with fundoplication by Yamashita(mSOFY);其改良之处是在食管右侧壁的开口通过旋转食管90°将食管右侧壁与残胃前壁吻合,关闭共同开口,而后在残胃前壁上将食管左侧壁缝合固定并使之紧贴残胃前壁;其研究数据表明,术后内镜下反流性食管炎洛杉矶分级A级和B级发生率均分别为7.1%,C级发生率为3.6%,总发生率约为17.9%。国内许多专家也在side overlap吻合的基础上进行了不同程度的探索和改良[52, 53, 54, 55, 56]。然而,关于此术式研究的样本量较少,仍需要更多的研究证据证实其有效性[57, 58]。
国内外多项研究对比了腹腔镜近端胃切除双通道吻合与腹腔镜全胃切除Roux-en-Y吻合两种消化道重建术式的疗效比较。樊林等[96]认为,胃切除双通道吻合用于早期胃上部癌的治疗安全可行,其术后短期营养状况及长期维生素B12水平优于全胃切除Roux-en-Y吻合;术后1年时,与全胃切除组患者相比,双通道吻合组患者的体质量、血红蛋白及维生素B12降低率更少,差异具有统计学意义(均P<0.05);术后2年时,与全胃切除组患者相比,双通道吻合组患者的维生素B12降低率仍更少(P<0.001)。此外,近端胃切除DTR吻合的患者预后良好,且发生严重反流、贫血和低蛋白血症等术后并发症的风险低于全胃切除组[97]。韩国一项前瞻性临床研究评估了LPG联合DTR(LPG-DTR)与腹腔镜全胃切除术(laparoscopic-assisted total gastrectomy,LTG)对胃上三分之一早期胃癌患者血红蛋白水平和所需的维生素B12补充水平的影响,该研究基于欧洲癌症研究和治疗组织生活质量问卷(European Cancer Research and Treatment Organization Quality of Life Questionnaire,EORTC QLQ)30项核心问卷(C30)和EORTC QLQ胃癌特异性问卷,结果表明,接受LPG-DTR治疗的患者所需的维生素B12补充剂量比接受LTG治疗的患者少,并发症发生率没有增加,总体生存率和无病生存率也没有差异;两组之间的血红蛋白水平变化没有差异;此外,LPG-DTR组的身体和社会功能优于LTG组[98]。德国的一项Meta分析纳入了13项临床研究,与全胃切除组相比,近端胃切除双通道吻合术在术后营养参数方面具有优势[99]。
